The CDMP covers 23 chronic conditions approved by MOH, such as diabetes, hypertension and lipid disorders. Our doctors can advise whether your specific condition qualifies under the programme and the type of care that can be claimed through MediSave.

You may use up to:

• S$500 per year for non-complex chronic conditions or
• S$700 per year for complex cases or when multiple CDMP conditions are managed together.

To use MediSave for your CDMP visits, you simply need to inform our staff during registration and provide the necessary details. The process is straightforward:

• Inform our staff during registration that you wish to use MediSave for CDMP.
• Present your NRIC and confirm your MediSave account details.
• Sign the required Medical Claims Authorisation Form (MCAF).
• Our clinic will then submit your MediSave claim electronically to the CPF Board for processing.

Once approved, the claim amount will be deducted

Yes. Immediate family members, such as your spouse, parents, or children, may use their MediSave to pay for your eligible CDMP expenses, provided proper authorisation is given.

If MediSave claims are not approved or are partially paid (for example, due to ineligible services), the remaining balance will need to be settled by cash, credit card or PayNow.

Being on a panel means your insurer has a direct billing arrangement with us. In most cases, we can submit your claim directly, reducing your upfront payment. You will only need to cover any co-payment deductible or non-claimable portion.

• Your insurance or corporate medical card
• Valid identification (NRIC or passport)
• Any referral letter or pre-authorisation (if required by your insurer)

We will issue you an itemised invoice and receipt so you can submit a reimbursement claim to your insurer. Coverage depends on your individual policy terms.

Yes. Some insurers exclude certain medications, investigations or specific treatments. Our staff will inform you if any service is not claimable before proceeding.

If your claim is rejected, contact your insurer to clarify the reason. We can provide medical details or supporting documents upon request, but the insurer makes the final decision. Any unpaid balance will need to be settled directly with the clinic.

We accept cash, major credit/debit cards and PayNow. For amounts not covered by MediSave or insurance, payment is due at the end of your visit.
